Registered Nurses

Supervisory Nurse assumes responsibility for patient assessment, patient care planning, provides case management, oversees the coordination of care and services, and provides field staff education, training, and supervision.

Licensed Practical Nurses 

The Licensed Practical Nurse (LPN) provides nursing care, treatments, medications and participates in the planning and coordination of patient care directed and supervised by the registered nurse as prescribed by the physicians in the plan of care. Carry out duties as prescribed by the physician, administer medication, and aid in performing specialized procedures.

Personal Care Aides 

The Home Health Aide (HHA) and Personal Care Aide (PCA) are trained paraprofessionals qualified to provide assistance with personal hygiene, dressing, meal preparation, feeding, toileting, ambulation, and transfers. The HHA provides certain specified Health-related tasks based on their training. Both HHA and PCA also provide assistance with incidental household tasks essential to the provision of home care services as prescribed by the physician and in accordance with the plan of treatment for the patient, supervised by the Supervisory Nurse.

TBI and NHTD Waiver Program Services 

Service Coordination- A Service Coordinator is chosen by the Waiver participant and/or guardian to work with identifying and coordinating support and services. 

Independent Living Skills Training- provides one-to-one support to focus on increasing independence in practical skills, such as cooking, shopping, banking etc. 

Home and Community Support Services- PCA or HHA that receives waiver service specific HCSS training and provides oversight and supervision, supportive services and assistance to enable the Waiver participant to reside safely within the home and community environments.
